Evidence of identity (EOI)
WorkCover NSW changed the evidence of identity requirements on July 1st 2008 see table. Also note in the event of a change of name such as marriage or divorce all forms of indemnity must be in the same name. There are special EOI requirements for
- Aboriginal or Torres Straight Islanders
- School Students
- People who arrived in Australia less than six weeks
- People in Correctional centres

From July 1st 2008 Evidence of Identity must be provided prior to training commencement
Trainers must not allow any person who cannot provide adequate evidence of identity to undertake training.

| Secondary Documents Allowed to use a combination of secondary documents. If you want to use credit or savings account cards, these must be from different banks. If you want to use more than one utility bill, they must be from different utilities. Mobile phone bills are unacceptable. |

The EOI documents provided by the participants must include:
a) Photo b) Date of birth c) Current address d) Signature
All EOI documents must be original. Certified documents are not to be accepted.
